Habilitation Home Care

Habilitation Home Care

This Service provides a variety of supports designed to increase a person’s independence.

Role Summary
Employee is responsible for supporting the client in gaining knowledge and skills, assisting in learning socialization skills and appropriate behavior as well as gaining and maintaining a quality life. This support may occur in the person’s home or in the community, based on the person’s specific needs, as identified by the Individual Support Plan process.

Some of the tasks related to Habilitation may include:

  • Assistance and training related to personal and physical needs and routine daily living skills
  • Implementing strategies to address behavioral concerns
  • Ensuring health needs are being met
  • Implementing therapy recommendations
  • Training in mobility or alternative or adaptive communication
  • Assisting in learning to use community transportation

Responsibilities
Develop teaching plans for each client’s objectives as per ISP with measurable outcomes noting individual’s ability to meet the objective.

The teaching plan will include a procedure that will be followed should the objective be completed. The teaching philosophy would embark on the premise that a short term objectives should work progressively toward a long term goal. The teaching plan should also include the methodology, materials, frequency of implementation, etc.

Must be Prompt and reliable

  • Give verbal reminders and or prompting as directed per ISP.
  • Actively participate in continuing skill development activities.
  • Attend other training per ISP or provider request.

Qualifications 

  • Strong verbal and written communication skills
  • Minimum of 3 months progressive hands on personal client care experience.
  • Minimum of 3 months of writing Teaching Plans/Strategies & Behavior plans.
  • CPR
  • First Aid
  • Article 9 training
  • Ability to interact with families, Support Coordinator and other ISP team members.
  • CIT (for DDD)

Hourly Attendant Care

Hourly Attendant Care

This service provides necessary support in order for the person to remain in His/her own home and/or participate in work or community activities.

Role Summary
Employee is responsible for assisting the consumer in maintaining a safe and clean home, stay healthy, clean and have good meals. This support may occur in the person’s home or in the community, based on the person’s specific needs, as identified through the Individual Support Plan process.

Some of the tasks related to Attendant Care may include:

  • Bathing
  • Tooth brushing
  • Using the toilet
  • Dressing
  • Hair washing
  • Eating and meal preparation
  • Transferring to or from a wheelchair
  • Planning and shopping for meals
  • Participating in community activities
  • Self-medication or medication reminders
  • Housecleaning and laundry
  • etc.

Responsibilities
Attendant Care/Housekeeping Agreement: responsible for being prompt and reliable. If you want to take time off, must give at least two week’s notice to make other arrangements for person’s care. Responsible for asking any questions to insure consumer’s care is appropriate. While you are performing personal care, notify consumer of any scrapes, bruises or skin problems.

Help consumer pay bills; drive to and from activities and doctor’s appointments, assist with lift on wheelchair vans and any other personal care equipment.

Give verbal reminders and or prompting as directed per ISP.
Actively participate in continuing skill development activities.
Attend other training per ISP or provider request.

Qualifications
Minimum of 3 months progressive hands on personal client care experience as well as some housekeeping.

Independent Living Arrangements

Independent Living Arrangements

This service provides necessary support in order for the person to remain in His/her own home and/or participate in work or community activities.

Role Summary
Is responsible for assisting the consumer in having a safe and clean home, stay healthy and clean and have good meals. This support may occur in the person’s home or in the community based on the person’s specific needs, as identified through the Individual Support Plan process.

Some of the tasks related to may include:

  • Driving/Transportation availability
  • Assisting person with emergency contact information
  • Scheduling appointments
  • Help with environmental modifications/accessibility.

Responsibilities
Employee must be prompt and reliable. If you want to take time off, must give at least two weeks notice to make other arrangements for person’s care. Responsible for asking any questions to insure person’s care is appropriate.

Respite Relief

Respite Relief

This service provides short –term care and supervision in accordance with the person’s Individual Support Plan. The goal is to provide a “break“ for the caregiver.

Role Summary
Employee is responsible for providing for the social, emotional and physical needs of the person.

  • Ensuring medication is taken as prescribed.
  • Providing appropriate first aid or attention to an illness or injury
  • Providing appropriate food
  • Following the person’s Individual Support Plan.
